Quarterly reports to practices on referrals for POLCV restricted activity

• Practices will receive quarterly reports on referrals for POLCV restricted activity via Moz (CCG Information Manager)at the following intervals
  • Quarter 1 : will be sent out in August 2012
  • Quarter 2 : November 2012
  • Quarter 3 : February 2013
  • Quarter 4 : May 2013
• The IFR team may contact practices directly in relation to these reports during the course of the year.
