  • Please note all minor surgery procedures are subject to the criteria and principles contained within the ELC POLCV Policy.
  • Please refer to this policy before referring patients to these services to ensure the referral is clinically appropriate and in line with the POLCV policy and referral criteria.

IF YOU ARE CONDUCTING THE MINOR SURGERY PROCEDURE IN HOUSE

  • Please check with the PCT Direct Commissioning Team for how the POLCV policy applies to minor surgery conducted under the MS LES on your own registered patients 'in-house'

IF YOU ARE REFERRING YOUR PATIENT TO A COMMMUNITY MS PROVIDER

  • The requirement exists for all GPs to use the 'POLCV pre-referral / pre procedure check form' before referring a patient to one of the 3 Community MS Providers
  • The POLCV checklist questions have been inserted at the top of the revised referral forms below:
